RN Wound Care & Ostomy Nurse provides expert clinical care for patients with complex wounds, ostomies, and skin integrity needs.
Key Responsibilities
- Conduct comprehensive patient assessments focused on wounds, ostomies, and skin health.
- Develop and implement evidence-based wound and ostomy care plans.
- Collaborate with physicians, therapists, and interdisciplinary teams to coordinate care.
- Educate patients and families on wound prevention, ostomy management, and self-care techniques.
- Document care and clinical findings thoroughly in the EMR.
Qualifications & Skills
- Active RN licensure.
- Strong clinical knowledge in wound care, ostomy care, and skin integrity management.
- Ability to assess complex conditions and apply evidence-based interventions.
- Excellent communication and patient/family education skills.
- Strong teamwork and coordination skills with interdisciplinary care teams.
- Detail-oriented documentation in the EMR.
