Role: Wellness Nurse (LPN/LVN) responsible for supporting resident health, monitoring care needs, and ensuring clinical compliance in a senior living setting.
Key Responsibilities:
- Conduct wellness assessments and monitor health changes of residents
- Support medication management, including MAR/TAR audits
- Communicate with families, physicians, and care teams to coordinate care
- Monitor incidents, falls, and follow-up care to ensure resident safety
- Maintain accurate documentation and ensure regulatory compliance
- Support infection control and clinical quality initiatives
- Train and assist care team members to uphold care standards
Qualifications & Skills:
- Active LPN/LVN license
- At least 1 year of experience in senior living or a clinical setting
- Strong assessment, documentation, and medication compliance skills
- Excellent communication and problem-solving abilities
