About the Role:

The Wellness Nurse (LPN/LVN) supports resident health and ensures clinical quality within a senior living environment. Key responsibilities include conducting wellness assessments, monitoring health changes, managing medications, and performing MAR/TAR audits. The role involves effective communication with families, physicians, and care teams, as well as documenting care activities to ensure compliance with regulations. The nurse is also responsible for incident and fall monitoring, supporting infection control efforts, and training care team members to uphold high-quality care standards.

Qualifications and Skills:

• Active LPN/LVN license
• At least 1 year of experience in senior living or a clinical setting
• Strong assessment, documentation, and medication administration skills
• Excellent communication and problem-solving abilities
