Key responsibilities:
- Communicate functional changes and barriers to the care team to support care planning
- Provide therapeutic interventions and monitor patient progress toward goals
- Evaluate functional status and develop treatment plans aligned with clinician and patient needs
- Support safe mobility using standardized safety practices during sessions
Role details:
- Work 40 hours per week with 8-hour shifts
