Travel Nurse Case Manager – Full Time

Role Overview:

We are seeking a Registered Nurse to provide in-home patient care as part of an interdisciplinary team, including Nurse Practitioners, Behavioral Health Advocates, and Care Navigators. The RN assesses health status, develops and evaluates care plans, and collaborates with other healthcare professionals to manage patient needs. Responsibilities include conducting assessments, counseling, educating patients and families, coordinating referrals and transitions, documenting in electronic medical records, and participating in quality initiatives. The role involves travel across the state to support growth, coverage, and caseload management.

Qualifications & Skills:

Unrestricted RN licensure in the practice state

At least 2 years of nursing experience, especially with complex behavioral, social, or functional needs

Ability to work with diverse care teams in various settings, including patients’ homes

Computer proficiency and valid driver’s license with the ability to travel up to 75%

Physical ability to lift up to 30 pounds and navigate home environments

Preferred Skills:

Experience with Electronic Medical Records

Strong time management, communication, and organizational skills

Effective communication with seniors and families
