Role Overview: Evaluates and provides speech therapy services addressing deficits in speech, language, cognition, voice, and dysphagia, primarily in a home setting.
Key Responsibilities:
- Conducts assessments using formal and informal diagnostic tools to determine patient needs.
- Develops and implements individualized treatment plans aligned with patient goals.
- Adjusts intervention strategies based on ongoing evaluations.
- Delivers direct therapy tailored to patients’ physical, emotional, and intellectual capabilities.
- Prepares documentation including evaluation results, treatment plans, and progress notes.
- Communicates effectively with patients, families, and healthcare team members.
- Collaborates with interdisciplinary teams and participates in program development and student internships.
Qualifications & Skills:
- Must hold CCC-SLP from ASHA or be actively completing a supervised clinical fellowship with certification within one year, or hold a state license.
- Preferred 1-3 years of experience in hospital, SNF, outpatient, or home health settings.
- Ability to work effectively, communicate clearly, and demonstrate compassion.
- Physical and visual acuity sufficient to perform job duties.
