Speech Pathologist – Live Well Homecare

Role: Speech Pathologist

Responsibilities:

Evaluate and treat patients with speech, language, cognitive-communication, and swallowing disorders

Design evidence-based treatment plans and provide individualized therapy

Document patient outcomes in the EMR

Collaborate with an interdisciplinary team to support patient care

Educate patients and families about treatment and progress

Assist with safe discharge planning and contribute to program development

Qualifications & Skills:

Strong communication and interpersonal skills

Ability to work collaboratively in a team environment

Knowledge of evidence-based practices in speech-language pathology
