We are seeking a compassionate *Speech-Language Pathologist (SLP)* to join our rehabilitation team in a Skilled Nursing Facility in Santa Rosa, CA.
Conduct comprehensive evaluations and develop individualized treatment plans for residents with speech, language, cognitive-communication, and swallowing disorders. Provide evidence-based therapy, including dysphagia management, communication, voice, and cognitive rehabilitation. Optional credentialing for specialized assessments like MBS or FEES. Complete accurate documentation in compliance with regulatory standards and track minutes for Medicare guidelines. Qualifications include a Master’s degree, CA SLP license, and preferred CCC-SLP or CFY experience.
