We are seeking a licensed Speech Language Pathologist (SLP) for a part-time/PRN role to assess, diagnose, and treat communication and swallowing disorders in patients’ homes.
Responsibilities include conducting initial assessments, developing individualized treatment plans, collaborating with the care team to ensure continuity of care, and documenting patient progress in the health information system.
Work involves addressing cognitive-communication and swallowing issues resulting from stroke, brain injury, illness, surgery, injury, or dementia.
Requirements:
- Current SLP license
- Valid driver’s license
- Ability to relocate to State College, PA before starting
