We are seeking a licensed Speech Language Pathologist (SLP) for a Part-time/PRN role to assess, diagnose, and treat communication and swallowing disorders in patients’ homes.
Responsibilities include:
- Conducting initial assessments
- Developing and coordinating individualized treatment plans
- Collaborating with the care team to ensure continuum of care
- Documenting patient progress in the health information system
