Provide in-home evaluation and treatment for adult and geriatric patients as a Speech Language Pathologist.
Key responsibilities
- Assess speech, language, cognitive-communication, and swallowing disorders
- Develop individualized treatment plans based on patient needs
- Deliver evidence-based therapy in patients’ homes
- Collaborate with an interdisciplinary care team
- Educate families and caregivers to support care at home
- Complete accurate, timely documentation and maintain required records
- Maintain quality, safety, and regulatory compliance
- Support patients in maintaining safe independence at home
Qualifications/skills
- Strong clinical assessment and treatment planning skills
- Expertise in evidence-based speech-language pathology interventions
- Excellent communication and family/caregiver education abilities
