Role Overview

This position involves providing comprehensive biopsychosocial/spiritual assessments, diagnoses, and interventions for diverse patient populations. The Social Worker collaborates with patients, support systems, and multidisciplinary teams to develop and implement evidence-based, person-centered care plans that respect patient self-determination and aim to reduce health disparities.

Key Responsibilities

  • Conduct biopsychosocial/spiritual assessments and formulate diagnoses or treatment plans.
  • Collaborate with patients, families, and healthcare teams to develop and execute care strategies.
  • Advocate for patients and support systems, ensuring ethical practice aligned with NASW standards.
  • Work with inpatient and outpatient populations across various departments.
  • Respect patient autonomy, promote equitable access to care, and adhere to ethical guidelines.

  • Qualifications & Skills

  • Master’s Degree in Social Work from an accredited program.
  • Licensure: Oregon Clinical Social Worker License or equivalent in care delivery state(s); for MSW roles, within 60 days of hire.
  • Relevant experience, including clinical or care management in healthcare settings preferred.
  • Strong assessment, communication, and collaboration skills.
