Role Overview
This position involves providing comprehensive biopsychosocial/spiritual assessments, diagnoses, and interventions for diverse patient populations. The Social Worker collaborates with patients, support systems, and multidisciplinary teams to develop and implement evidence-based, person-centered care plans that respect patient self-determination and aim to reduce health disparities.
Key Responsibilities
Conduct biopsychosocial/spiritual assessments and formulate diagnoses or treatment plans. Collaborate with patients, families, and healthcare teams to develop and execute care strategies. Advocate for patients and support systems, ensuring ethical practice aligned with NASW standards. Work with inpatient and outpatient populations across various departments. Respect patient autonomy, promote equitable access to care, and adhere to ethical guidelines. Qualifications & Skills Master’s Degree in Social Work from an accredited program. Licensure: Oregon Clinical Social Worker License or equivalent in care delivery state(s); for MSW roles, within 60 days of hire. Relevant experience, including clinical or care management in healthcare settings preferred. Strong assessment, communication, and collaboration skills.
