Social Worker – Community Health Initiatives

The Social Worker provides professional psychosocial services to clients and families eligible for Healthy Beginnings and Mobile Health Clinic programs.

Responsibilities include conducting assessments, offering supportive counseling, crisis intervention, facilitating care transitions, and referring clients to community resources. The role involves collaborating with interdisciplinary teams, documenting interactions, developing care plans, and providing education on health and life issues.

Qualifications required are a Master’s or Bachelor’s Degree in Social Work, relevant work experience, and appropriate licensure or certifications such as LSW, LCSW, or ACSW.

Key skills include client advocacy, system-level intervention, community engagement, documentation, and crisis management. The position also involves developing community relationships, supporting high-risk populations, and maintaining data systems to enhance service delivery.

Additional duties may include insurance audits and patient support training. The role aims to promote positive health outcomes through holistic, patient-centered care within a collaborative team environment.
