Provide assessments, diagnosis, brief therapy, and discharge planning for patients from birth to elder, supporting care transitions and family wellness.
Coordinate with hospital teams and community partners, advocate for clients, and assist with complex matters like Power of Attorney.
Ensure accurate documentation in compliance with regulations, complete assessments, treatment plans, and reports on time.
Participate in team meetings, contribute to treatment planning, and support staff training initiatives.
Require a Master’s in Social Work, Psychology, Counseling, or related field; licensure or progression toward licensure (LCSW, LPC, LMFT); clinical competency within 3 months.
