Social Services Clinician – In-Patient – Discharge Planning

Seeking a skilled Social Services Clinician for a Level IV Critical Access Hospital in Southeast Alaska.

Responsibilities include conducting assessments, brief therapy, and discharge planning for patients from birth to elder, with a focus on care coordination, advocacy, and cultural wellness.

Provide clinical support for mental health and substance use, facilitate patient care conferences, and ensure accurate documentation in compliance with regulations.

Collaborate with hospital teams and community partners to optimize patient outcomes, participate in team meetings, and assist with staff training.

Qualifications include a Master’s in Social Work, Psychology, or related field, licensure or pending licensure as an LCSW, LPC, or LMFT, and clinical competency within 3 months.
