We are seeking a qualified Nurse Practitioner (Skilled Nursing) to join our team in a Skilled Nursing Facility setting.

Serve as a clinical leader, collaborating with the care team to oversee residents’ short- and long-term needs.

Perform admissions, discharges, monthly stability checks, and assessments for condition changes.

Handle advanced care planning, comprehensive evaluations, and prescribe treatments.
