Senior Nurse Practitioner – Skilled Nursing Facility – Altira at Inners Creek

We are seeking a qualified Nurse Practitioner (Skilled Nursing) to join our team in a Skilled Nursing Facility setting.

As a clinical leader, you will collaborate with the care team to oversee residents’ short- and long-term needs, including admissions, discharges, assessments, and evaluations for changes in condition.

You will handle advanced care planning, conduct comprehensive assessments, and prescribe treatments.

Requirements:

Active Pennsylvania Advanced Practice Licensure (APN) and DEA
