Role Overview
In this position, you assess and develop personalized plans of care to help patients recover from illness or injury in their homes. You perform initial and ongoing evaluations of mobility, strength, and function, interpret test results, and set measurable goals. You deliver prescribed physical therapy services, document progress accurately, and modify treatment plans as needed. You collaborate with healthcare team members, educate patients and caregivers on home exercises, and supervise other personnel such as PTA, OTA, or HHA, where applicable. The role includes completing OASIS assessments within scope of practice and ensuring compliance with policies, regulations, and standards.
