Role Overview: This position involves serving as an expanded nursing role within an interdisciplinary team to promote preventative care, conduct screenings, and coordinate disease management services. The nurse provides primary clinical support to providers, patients, and families, assisting in diagnosis, treatment, and care planning. Responsibilities include triaging patient interactions, identifying needs, and developing individualized care plans based on clinical guidelines and risk assessments.
Key Duties: Assess healthcare, educational, and psychosocial needs; facilitate communication among healthcare team members; educate patients and families on disease and case management; implement clinical interventions; collaborate with providers to ensure guideline adherence; coordinate diagnostic tests; maintain documentation; participate in ongoing professional development; assist with discharge transitions; support care coordination program development; and administer medications and vaccines within scope of practice.
Qualifications & Skills: Minimum of 2 years RN work experience; graduate from a specialty nursing training program (required), bachelor’s degree preferred. Strong clinical, communication, organizational, and collaboration skills essential for success.
