Role Overview: This position is responsible for coordinating and delivering infusion therapy services across clinical settings, including home and hospital environments. The nurse manages referrals, verifies clinical and administrative requirements, provides patient education, and collaborates with interdisciplinary teams to ensure safe, effective, and patient-centered care.
Key Responsibilities:
- Coordinate intake, referrals, and develop individualized care plans based on patient assessments.
- Administer infusion medications, perform vascular access procedures, and conduct lab draws using aseptic techniques.
- Educate patients and caregivers on therapy protocols and equipment use.
- Monitor therapy progress, adjust care plans, and communicate with providers for optimal outcomes.
- Ensure documentation accuracy, compliance with standards, and participate in quality initiatives.
- Support 24/7 care coverage through on-call rotations and travel to patient homes and clinical sites.
- Provide clinical oversight to Licensed Practical Nurses and contribute to protocol development.
Qualifications & Skills:
- Minimum of three years RN experience required; home health and IV therapy experience preferred.
- Graduate from Specialty Training Program-Nursing (Required).
- Strong communication, collaboration, and clinical skills, including vascular access expertise.
- Ability to travel and perform in diverse clinical environments.
