Role Overview:
This position is for a PRN Home Health Registered Nurse responsible for providing skilled nursing services in patients’ homes, including assessments, care planning, and implementation. The nurse evaluates patient needs, develops individualized care plans, and collaborates with a multidisciplinary team to ensure quality, evidence-based care that respects patient preferences and cultural backgrounds. Responsibilities include monitoring patient conditions, updating care plans, and communicating effectively with patients, families, and team members. The role involves working in a safety-sensitive environment, requiring alertness and safe practices.
Qualifications & Skills:
- Nursing Diploma or Associate’s Degree in Nursing (RN license required)
- Minimum of 2 years of experience; experience in home health is preferred
- Valid Driver’s License
- Excellent critical thinking, communication, and cultural competency skills
- Ability to work independently and collaboratively
