Role: RN Clinical Care Coordinator, responsible for managing a panel of members with complex medical and behavioral needs.
Key Duties: Conduct comprehensive needs assessments, develop and implement person-centered care plans, collaborate with care teams and community resources, provide member education and coaching, support discharge planning, and advocate for member needs.
Qualifications: Current RN license in MA, 2+ years clinical experience, proficiency in MS Office, reliable home workspace, high-speed internet, transportation, and valid US driver’s license.
Preferred Skills: Bachelor’s or Master’s in Nursing, CCM certification, community case management experience, familiarity with electronic health records, and virtual care platforms.
Physical Requirements: Ability to travel within the territory, navigate various locations, transport equipment, and remain stationary for tasks.
