Case Manager RN coordinates rehabilitation and acute care services to ensure safe, patient-centered care transitions.
- Guide patients and families through program orientation, benefits, and patient rights.
- Complete psychosocial assessments and use findings to support individualized care planning.
- Lead and manage discharge planning, coordinating resources to meet each patient’s needs.
- Develop and implement individualized care plans in collaboration with physicians, nursing, and case management leadership.
- Promote patient and family education aligned with treatment goals and transition of care.
Qualifications/Skills: Registered Nurse with strong assessment, care coordination, discharge planning, and communication skills, with the ability to work effectively in multidisciplinary teams and support patient-centered outcomes.
