Summary of Home Healthcare Nursing Role

This position involves providing comprehensive patient care in a home setting, including case management, post-hospitalization assessments, discharges, patient teaching, dressing changes, IV therapy, access site care, and general skilled nursing according to policies and procedures.
It requires timely documentation using the Epic system and may include assisting with safety needs and public program instructions to ensure appropriate care.

The role is per diem, with visits scheduled as needed, adhering to department policies and organizational standards.

Qualifications & Skills

• Graduation from an accredited nursing school
• Current state licensure and valid driver’s license
• CPR certification
• Preferred: at least one year of related experience
