Join a supportive and collaborative nursing team in a community-based care unit, providing exceptional patient care in a stable day-shift setting. This role is open to both new graduate and experienced Registered Nurses with ADN, BSN, or MSN degrees.
Key responsibilities include:
- Assessing patient needs, preferences, and barriers to involvement in care, including cultural, emotional, and spiritual factors.
- Developing, implementing, and documenting individualized care plans in collaboration with the interprofessional team, patients, and families.
- Promoting respectful, equitable, and empathetic interactions with diverse populations, ensuring continuity of care, and practicing collaborative problem solving.
- Evaluating patient condition changes, updating care plans, and communicating effectively with team and caregivers.
- Engaging in professional development activities to advance clinical practice and contribute to quality improvement initiatives.
Qualifications include:
- Nursing diploma, Associate’s, Bachelor’s, or Master’s degree in Nursing.
- RN license.
- Minimum of 2 years experience preferred; no supervisor experience required.
