Registered Nurse (RN) – Acute Care – Care Coordination Senior

Seeking a Care Coordination RN Senior to assess, plan, and coordinate care for complex patients during acute hospital stays, primarily in the Emergency Department.

Responsibilities include conducting transitional care planning, evaluating psychosocial and functional needs, collaborating with physicians and interdisciplinary teams, and facilitating timely discharge and post-acute referrals. The role involves managing discharge processes, documenting assessments and care plans, and mentoring new staff and students.

Qualifications require a minimum of

3 years of acute care nursing experience

and

2 years in hospital-based care coordination or case management.

Preferred qualifications include a Bachelor’s in Nursing and relevant certifications such as BLS and RN licensure.
