Registered Nurse Navigator Triage – Population Health

Role Overview: The RN Nurse Navigator Triage provides nursing triage services and health advice via phone and in person, assessing patient acuity and determining medical needs. Responsibilities include documenting patient information, providing protocol-driven clinical advice, coordinating emergency and routine care, and educating patients and families. The role involves scheduling appointments, facilitating communication across departments, supporting new staff, and ensuring timely documentation. The position emphasizes patient satisfaction, care coordination with external organizations, and continuous quality improvement.

Qualifications & Skills: A graduate from a specialty nursing program is required, with a preference for a Bachelor’s Degree in Nursing. Five years of nursing experience, especially in ICU, ED, or Critical Care, is strongly preferred. Proficiency in computer skills and Microsoft Office Suite is essential.

Key Duties:

  • Perform patient assessments over the phone, documenting symptoms, history, and contact info.
  • Initiate emergency treatment when needed and prioritize urgent calls.
  • Provide patient education, schedule appointments, and coordinate care across departments.
  • Support onboarding of new staff and ensure effective communication with providers and patients.
  • Explain medical conditions, manage patient concerns, and reinforce treatment instructions.
  • Ensure accurate documentation and suggest process improvements.
  • Assist with external care coordination, including insurance and medical facilities.
