Job Summary: Provides nursing triage services and health advice via phone and in person, assessing patient acuity and determining medical support needs. Ensures continuity of care by delivering diagnostic results, education, and protocol-driven clinical advice, while maintaining accurate documentation and timely communication.
Responsibilities include:
- Performing patient assessments, documenting symptoms, history, and contact information.
- Initiating emergency treatments and prioritizing urgent calls.
- Providing patient and family education, scheduling appointments, and coordinating care across departments.
- Supporting onboarding of new staff and managing ongoing patient communication.
- Explaining medical conditions, managing patient concerns, and ensuring understanding of treatment plans.
- Collaborating with external organizations and assisting with care coordination.
Qualifications & Skills:
- Graduate from Specialty Training Program-Nursing required; Bachelor’s Degree preferred.
- 5+ years nursing experience, with ICU/ED/Critical Care experience strongly preferred.
- Proficiency with computer systems, including Microsoft Office and telehealth platforms.
- Specialty certification is a plus.
Key Duties: Deliver patient-centered triage, coordinate emergency and routine care, educate patients, document interactions, and support continuous improvement efforts within a clinical environment.
