Qualifications & Skills: Requires a minimum of 2 years related nursing experience. Education includes graduation from a specialty training program; a Bachelor’s Degree in Nursing is preferred. Strong communication, coordination, and patient education skills are essential.
Work Environment & Additional Duties: Performs in a clinical setting, maintaining compliance with policies. Supports patient education, monitors follow-up processes, and educates team members. Coordinates support services such as social support and support groups to meet patient needs.
