Role Overview: This position involves coordinating patient care within multidisciplinary programs, serving as a liaison between patients and healthcare providers, and facilitating access to care. The role includes patient education, promoting understanding of conditions, and ensuring continuity of care through effective communication and collaboration.
Key Responsibilities:
- Facilitates entry of new patients into care programs and coordinates pre- and post-hospital care.
- Acts as a patient navigator, guiding patients through their care journey and coordinating multidisciplinary clinic activities.
- Provides clinical support to providers and patients for diagnosis and treatment.
- Communicates with patients via mail, phone, and in person, and coordinates support services like social programs and support groups.
- Maintains systems for timely follow-up and surveillance of patients.
- Provides nursing care, assists physicians, and educates team members in specialized areas.
Qualifications & Skills:
- Graduate from Specialty Training Program (required); Bachelor’s Degree in Nursing preferred.
- Strong communication, organizational, and patient advocacy skills.
- Ability to collaborate effectively within healthcare teams.
