Role Overview:
This is a part-time nursing position requiring evaluation, care planning, and coordination for home care patients within the Southeast Region. The nurse assesses patient needs, develops and documents personalized care plans, and collaborates with an interprofessional team to ensure quality, culturally sensitive care that respects patient preferences.
Key Responsibilities:
- Assess patient preferences, barriers, and needs across emotional, spiritual, cultural, and health domains.
- Create and implement individualized care plans with clear goals, ensuring communication across care settings.
- Promote respectful, equitable, and empathetic interactions with diverse populations.
- Evaluate patient condition changes, update care plans, and collaborate with family and team members.
- Ensure evidence-based practice and safe, timely care delivery.
Qualifications & Skills:
- Nursing diploma or associate degree in nursing.
- Valid RN license and driver’s license.
- Minimum of 2 years nursing experience; preferred Bachelor’s in Nursing/Home Health and 2-5 years of experience.
- Strong communication, critical thinking, cultural competence, and teamwork skills.

- Willingness to provide coverage across regions during on-call rotations.
- Registration with the Family Care Safety Registry.
