Role Overview:
This position involves providing specialized nursing care through home visits, assessing patient needs, developing personalized care plans, and collaborating with an interprofessional team to ensure quality outcomes. Qualifications & Skills:
- Minimum of a Nursing Diploma/Associate’s degree in Nursing
- Valid RN license and driver’s license
- At least 2 years of nursing experience; preferred Bachelor’s degree in Nursing or Home Health with 2-5 years of experience
- Strong critical thinking, communication, and cultural competency skills
- Ability to work safely and efficiently in a safety-sensitive environment
Key Duties:
- Assess patient needs and barriers to care, including emotional, spiritual, and cultural factors
- Develop, implement, and document individualized care plans with measurable goals
- Coordinate care across the continuum, ensuring effective communication and collaboration
- Monitor patient condition changes, update care plans, and communicate with caregivers and the team
