Registered Nurse – Graduate – Live Well Homecare

Role Overview: This position involves assessing, planning, and providing comprehensive nursing care to patients, ensuring safety, comfort, and satisfaction. The nurse promotes adaptive responses, coordinates interdisciplinary care, and participates in decision-making to improve patient outcomes.

Responsibilities include:

Conducting assessments and developing individualized care plans

Delivering direct patient care, including treatments, medications, and diagnostics

Evaluating intervention effectiveness and modifying plans accordingly

Collaborating with physicians and healthcare team members to address patient needs

Problem-solving equipment issues and participating in evidence-based practice initiatives

Engaging in quality, safety, and departmental improvement activities

Qualifications and Skills: A graduate of a specialty nursing training program, with strong assessment, communication, and teamwork skills. Ability to provide compassionate, culturally sensitive care and adapt to evolving clinical situations.

Key Duties: Ensure optimal patient and family experiences, participate in patient education and discharge planning, and uphold organizational policies. Support professional growth through mentorship and development programs, including a nurse residency for those with less than one year of acute care experience.
