Role Overview: This position involves serving as an expanded nursing role within an interdisciplinary team to promote preventive care, conduct screenings, coordinate disease management, and support clinical decision-making. The nurse will assess patient needs, facilitate communication among healthcare providers, educate patients and families, and implement clinical interventions based on evidence-based guidelines.
Responsibilities include:
- Assessing healthcare, educational, and psychosocial needs to develop personalized care plans.
- Investigating healthcare options and coordinating communication among patients, providers, and team members.
- Educating patients and families on disease management, care coordination, and treatment goals.
- Implementing clinical interventions, managing diagnostics, and maintaining documentation in electronic health records.
- Monitoring therapies, collaborating with providers, and facilitating follow-up care.
- Participating in professional development, reviewing current literature, and supporting care transition programs.
- Assisting with laboratory procedures, medication administration, IV management, and vaccine delivery within scope of practice.
Qualifications & Skills: RN with at least two years of experience, preferably in Med/Surg settings. Must be a graduate of a Specialty Training Program in Nursing; a Bachelor’s Degree is preferred. Strong communication, clinical judgment, and collaboration skills are essential.
Key Duties: Provide patient-centered clinical support, coordinate care activities, promote adherence to clinical guidelines, and contribute to quality improvement initiatives. Work typically in a clinical environment, ensuring compliance with organizational policies.
