Registered Nurse Coordinator – Primary Care

Role Overview: This position involves serving as an expanded nursing role within an interdisciplinary team to promote preventative care, conduct screenings, and coordinate disease management. The nurse provides primary clinical support to patients, families, and providers, assisting with diagnosis, treatment, and care planning. They assess healthcare, educational, and psychosocial needs, facilitate communication among care team members, and educate patients on disease management and care coordination.


Key Responsibilities:

  • Implement clinical interventions based on risk stratification and evidence-based guidelines.
  • Collaborate with providers to enhance guideline adherence and promote best practices.
  • Coordinate diagnostic tests and manage documentation within electronic health records.
  • Assist in post-discharge transitions of care and support care coordination programs.
  • Administer medications, vaccines, and IV therapies as directed within scope of practice.
  • Investigate healthcare options, eliminate barriers, and promote patient engagement.
  • Maintain professional development through continuing education and literature review.

Qualifications & Skills:

  • Graduate from a Specialty Training Program in Nursing required; Bachelor’s Degree preferred.
  • At least two years of RN experience, preferably in Med/Surg or hospital settings.
  • Strong clinical judgment, communication, and teamwork skills.
  • Proficiency in clinical documentation and patient education.
