Role Summary
Provide patient-centered nursing care in a Clinical Transition Unit. Assess patients and monitor vital signs and condition, document findings accurately, and perform ordered procedures and treatments. Administer medications safely and respond promptly to patient needs and changes in status.
Educate patients and their families on treatment plans, procedures, and follow-up care to support understanding and continuity of care.
