Role Summary: Provide patient-centered nursing care in a Clinical Transition Unit by assessing patient conditions, administering medications, and performing ordered procedures. Document nursing care accurately and collaborate closely with healthcare providers to ensure safe, positive clinical outcomes.
Key Duties:
- Assess patients’ conditions and respond to clinical changes.
- Administer medications and complete ordered nursing procedures.
- Document care and patient status in accordance with standards.
- Coordinate with providers and the care team to support clinical continuity.
- Educate and advocate for patients and families.
- Maintain a focus on safety, patient experience, and positive outcomes.
