RN Case Manager (PRN)
Registered Nurses are sought to coordinate comprehensive, patient-centered care. The RN Case Manager develops and manages care plans tailored to patients’ health needs, helping ensure appropriate and cost-effective treatment.
Key Duties
- Assess patient needs and coordinate individualized care plans
- Coordinate services across the care continuum to support continuity of care
- Facilitate access to appropriate, cost-effective treatment options
- Monitor patient progress and adjust care plans as needed
- Collaborate with clinical teams to support effective care delivery
Qualifications & Skills
- Active RN licensure
- Strong care coordination and case management skills
- Excellent clinical judgment and patient assessment abilities
- Effective communication and collaboration with multidisciplinary teams
- Ability to manage complex care needs efficiently
