Primary Care Nurse
Provide patient-centered nursing care in a primary care setting for diverse populations. Perform comprehensive patient assessments, develop and update care plans, administer medications, and educate patients and families to support informed decision-making.
Key Responsibilities
- Deliver high-quality clinical care through assessment, care planning, medication administration, and ongoing monitoring
- Coordinate with physicians and an interdisciplinary team to manage chronic conditions
- Support preventive care and wellness initiatives
- Ensure safe transitions of care and continuity across care settings
- Provide culturally responsive patient education and guidance
