Responsibilities:
Provide comprehensive adult primary care, including diagnosis, treatment, and personalized care plans.
Deliver preventive services such as screenings and vaccinations.
Utilize data to support value-based care, identify care gaps, and improve outcomes.
Collaborate with interdisciplinary teams for coordinated, high-quality care.
Maintain accurate EHR documentation and engage in practice innovation to meet clinical goals.
Qualifications:
MD or DO with active US medical license.
Experience in adult primary care, chronic disease management, or related fields.
Strong communication skills and interest in value-based care are preferred.
