Role Summary: Provide comprehensive outpatient primary care to older adults within a team-based, value-based care practice.
Key Responsibilities:
- Deliver high-quality outpatient primary care for older adult patients.
- Collaborate with the care team to engage patients and support shared care planning.
- Use coaching and integrated behavioral health resources to address patient needs.
- Leverage electronic health records to document, coordinate, and manage care.

Qualifications:
- Current Washington licensure.
- Board certification in Internal Medicine or Family Medicine.
