Primary Care RN/LPN—Provide compassionate, patient-centered primary care within a Healthcare & Medical Services team.
Key Responsibilities
- Perform patient assessments and support preventive care and wellness initiatives
- Coordinate patient care with an interdisciplinary team
- Administer medications and support safe medication management
- Educate patients on conditions, treatment plans, and self-management
- Support chronic disease management and help optimize patient outcomes
Qualifications & Skills
- Current RN or LPN licensure
- Strong clinical judgment, assessment, and patient education skills
- Ability to collaborate effectively with interdisciplinary teams
- Compassionate, patient-centered approach and commitment to community health
