Role Overview:
Provides direct patient care in an outpatient setting, collaborating with providers and the care team to develop and implement individualized care plans aimed at delivering high-quality, cost-effective outcomes. The RN offers oversight and guidance to clinical staff, supports daily patient management, and assists providers with documentation and patient education.
Responsibilities:
- Develop and update care plans with PCPs and specialists to enhance healthcare outcomes.
- Assess patients’ physical, psychosocial, and emotional needs using critical nursing skills.
- Ensure clinical protocols and assessment guidelines are followed for compliance and quality.
- Support documentation of patient assessments and treatments according to standards.
- Educate patients and families verbally and in writing, tailoring support to their needs.
- Work in a safety-sensitive environment, maintaining constant alertness and safety awareness.
Qualifications:
- Diploma or Associate’s degree in Nursing; RN licensure.
- 2-5 years of nursing experience; supervisory experience not required.
- Certifications: BLS required.
- Preferred: Bachelor’s degree in Nursing and 5-10 years of experience.
