Post Acute Care Physician / Primary Care Physician – Providence, RI

Seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living settings.

Providing care for a range of patients from short-term rehab to complex geriatric cases, with a focus on removing barriers, improving access, and delivering state-of-the-art care to achieve optimal outcomes.

As a team member, you’ll deliver face-to-face primary care, participate in educational sessions, and collaborate with nurse practitioners and physician assistants.
