Seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living facilities.
- Deliver face-to-face primary care services to diverse patient populations, from short-term rehab to complex geriatric cases.
- Participate in monthly educational sessions and clinical outcomes reviews.
- Collaborate with Nurse Practitioners and Physician Assistants to optimize patient care.
