Seeking Board Certified Geriatricians, Internists, and Family Practitioners to join a dedicated team providing comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living facilities.
This role involves delivering face-to-face primary care, participating in educational sessions, and collaborating with nurse practitioners and physician assistants. The focus is on improving access, removing barriers, and achieving optimal clinical outcomes across a spectrum of complex geriatric patients.
