Seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site care in post-acute, rehabilitation, long-term care, and assisted living facilities.
Our team delivers care across a spectrum from short-term rehab to complex geriatric patients, aiming to improve access, remove barriers, and achieve optimal clinical outcomes.
As a member, you will:
- Provide face-to-face primary care services
- Participate in educational lectures and outcomes reviews
- Collaborate with Nurse Practitioners and Physician Assistants
