Seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living facilities.
Deliver face-to-face primary care services and collaborate with healthcare teams. Participate in monthly educational sessions and clinical outcomes reviews. Work within a structured care model to enhance facility performance.
