We are seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living settings.
Deliver face-to-face primary care services to a diverse patient population, from short-term rehab to complex geriatric cases. Participate in monthly educational sessions and clinical outcomes reviews. Collaborate with Nurse Practitioners and Physician Assistants.
