Post-Acute and Long-Term Care (PALTC) – Wound Care Advanced Practitioner (WCAP)

The Nursing Home Wound Care Advanced Practitioner (AP) is a skilled clinical professional, such as a Nurse Practitioner or Physician Assistant, specializing in wound assessment, treatment, and management within a long-term care facility.

  • Perform comprehensive assessments and develop individualized wound care plans.
  • Administer advanced wound treatments, including debridement and specialized dressings.
  • Collaborate with interdisciplinary teams to ensure integrated wound prevention and care strategies.
  • Implement evidence-based prevention protocols and educate staff, residents, and families.
  • Maintain detailed documentation and participate in quality improvement initiatives.
  • Serve as a clinical resource, address infections, and ensure regulatory compliance.
  • Advocate for residents’ dignity and autonomy in wound care decisions.
  • Utilize electronic health records and telemedicine tools effectively.

Qualifications include a relevant degree, at least 2 years of experience, board certification (AANP/ANCC/PA-C), and preferred specialty certification in wound care. The role requires excellent communication, physical stamina, and the ability to build strong team relationships. This full-time position offers comprehensive benefits and involves travel within the facility. Licensure as an APRN or PA is required.
